Good idea to help delivering medicine to sick people who are not able to go to the pharmacy. But at this stage too early, since the community is not engaged to connect, the excisting healthcare system is not integrated, yet, to ensure that the delivery is sustain. Especially in non-urban environments is has to be ensured that patients will get help (due to high transport costs). The communication between patients & HCPs should be ensured. If the Take Care Team will go further steps, this will be an interesting platform.

Lots to like here, but probably too early stage.    They admit to not having impact data yet, and that's going to hold it back.    Currently it's on a small scale, connecting pharmacies with vulnerable patients via volunteer cyclists, but they have a clear plan to integrate more of the health system. They just haven't done it yet.    Definitely one to watch in the future and keep engaged

Francesca´s digital platform is supporting people living with disabilities with specialized training. A more open space to communities, to allow their members to connect and to find/share their needs to improve their situation would be great. The Community Buy-in has room for improvement. The platform could help to gain data to help finding & amplifying the needs of the people living with disabilities and their caregivers. But this seems not to be the plan (yet). The platform is a great tool to reach people inclusivly and for affordable costs. Would be great to consider working with existing healthcare stakebrokers to even lower the barriers.

This is predominantly a digital solution, not grounded in a physical community. However, as it's for disabled people I see the value in a digital solution like this.    They don't about health improvements, and there's minimal collab with health professionals

Novel Health Solution 

Francesca offers help for adolescents suffering from social withdrawal and their families in Italy. She (and a team) get in touch with the adolescents in a way they are used to and make the help therefore accesible (via online platform). It is a community for adolescents with similar challenges and interests and they are encouraged to help others after they tackled their own challenges. Parents and other stakeholders are trained, too.  The initiative has a truly community buy-in and impact. For the next steps it will be key to collaborate with key stakeholders to build a sustain network, business plan and to gain and exchange key learnings and data to share information to enhance the help. By gaining more data and qualitative feedback it would be benefitial to analyse all the data to adjust and enhance the program and create a learning system of evolving needs and challenges of the adolesctens and their families.  I think that this initiative is really worth to support and that the team could need some help to connect it to existing systems, KPI & feedback reporting and help setting it up to make it scalable.
